Schedule:  Day Shift/Monday – Friday

 

GENERAL STATEMENT OF DUTIES: Coordinate and facilitate Family Team Meetings with

Portage County Job and Family Services.

 

ESSENTIAL RESPONSIBILITIES:

1. Engage families, their supports, and the workers in fulfilling the purpose and the process of the Family Team Meetings.

2. Apply strategies to plan for and manage conflict during the Family Team Meeting process.

3. Establishes trusting relationship with participants.

4. Orchestrate movement through the stages of the meeting, employing communication and facilitative strategies for managing group processes and breaking impasse.

5. Apply various strategies that help the family identify and use their strengths in the process and conduct the Family Team Meeting in ways that are respectful of families’ attitudes, culture, and beliefs.

6. Maintain a complete family record for each client with who they engage, including all pertinent documents such as action plan, Family Team Meeting summaries, Semi-Annual Review documentation, case notes, and any assigned documentation and/or data used in evaluations and planning.

7. Facilitate and conduct Family Team Meetings, Semi-Annual Review (SAR), and Match Meetings per Ohio Administrative Code (OAC) requirements and timelines.

8. Guide meeting participants through discussions of concerns, brainstorming the identification of strengths that enlist relevant information to reach consensus, that supports making safety decisions.

9. Support the family in the meetings and in preparing for them.

10. Prepare accurate, timely, written documentation summarizing each Family Team Meeting and provide a documented action plan.

11. Collect and monitor data on individual cases and assist in developing and collecting aggregate data, including surveying participants in the Family Team Meeting, for evaluation and planning purposes.

12. Enter assigned documentation into SACWIS at the conclusion of each SAR and Match Meeting and review and scan documents received into Traverse within three (3) business days of the corresponding meeting.

13. Sign and comply with Ohio Department of Job and Family Services Code of Responsibility to use PCJFS State computer system.

14. Implements case management and supportive services, per agency and program policies and procedures.

15. Models awareness and behavior needed for growth and development.

16. Networks with area services, understands eligibility requirements, and makes appropriate referrals

17. Assists in creating and maintaining positive program image in community.

18. Attends agency and team meetings as requested.

19. Assists with program outreach, application, orientation, and approval process.

20. Fulfills other duties as assigned.

21. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements
Requirements
QUALIFICATIONS:

1. Ability to model open communication, fair and equitable treatment, and management of conflicts.

2. Ability to work as a team member.

3. Experience in utilizing community resources.

4. Valid Ohio Driver’s License and ability to meet agency requirements for driving insurability.

5. Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).

 

MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Bachelor’s Degree in Human Services field. Licensed Social Worker (LSW) preferred.

 

MINIMUM EXPERIENCE REQUIREMENTS: Minimum two years related experience. Experience providing supportive services (case management, crisis intervention, family development) for at-risk families.
